Concordia Home and Community Services are comprised of three organizations that offer a continuum of care in our patients’ homes.

Concordia Visiting Nurses is a non-profit home health agency based in Cabot, PA that has been offering Professional Home Health and Private Duty Home Care since 1997. We currently serve Allegheny, Armstrong, Beaver, Butler, Lawrence, Mercer and Westmoreland counties, and are expanding our service area every day.

Additionally, Good Samaritan Hospice and Concordia Medical Equipment deliver quality home health services and products to those who entrust themselves to our total care.

Our team includes registered nurses, medical social workers, physical, speech, and occupational therapists, intravenous and wound care nursing and home health aides as well as medical equipment technicians, respiratory therapists and more. All of these individuals are backed by an administrative team to provide the best service and products in your own home.
