Home Care - Medical Equipment
Salaried position with benefits including retirement, health, dental, and vision coverage. Experienced RN needed to provide direct nursing care to assigned patients within Concordia facilities and surrounding area. Coordinates total patient care, serving as the primary nurse for designated patients and evaluating the effectiveness and the appropriateness of care. Plans with the patient, family and other team members for the patient’s discharge from service.
RN intake coordinator accepts and records essential information about referrals from hospitals, nursing homes, physicians, families and patients, assigns primary staff, and provide referral copy to appropriate disciplines.
Pay per visit with flexible hours, mileage reimbursement and premium rates for evening, weekends and holidays. Manage client cases, home health aide supervisory and evaluations. Qualifications include current license, strong assessment and communication skills.
This individual must be motivated, detail oriented, and team focused. The ideal candidate would possess experience in conducting background checks, familiar with UC claims, Workers Compensation, FMLA, benefit enrollments, Word and Excel programs. Salary is dependant upon education and experience. Degree or certificate in Human Resources plus experience preferred.
Schedule therapy visits using Horizon system, professional appearance, communication and interpersonal skills, must be efficient, organized and able to multi-task, must have good computer and telephone skills, comfortable working closely with clinicians, and must be able to work weekends
Supervises field staff along with scheduling, disciplinary issues and reports, maintains schedules for all staff, receives referral information, and disseminates to the appropriate personnel, identifies problems/concerns, tracks activities of field staff. Coordinator ensures that services are scheduled and delivered in an accurate and timely manner. Assist with new business initiatives and recruitment has strong computer, organizational and communication skills. Experience in a health related field or post secondary education preferred.
Personal Care Aides needed immediately to assist clients with activities of daily living throughout Butler, Northern Allegheny and Armstrong Counties. We offer a competitive wage along with paid travel time and retirement benefits. Must have reliable transportation.
