Following an acute hospital stay, Concordia can provide skilled nursing/inpatient short-term rehab in one of our 242 beds in the Concordia system, the majority of which are in private rooms. Patients take comfort in knowing that our nursing staff is experienced in:

• Orthopedics
• Wound care
• Cardiac care
• Intravenous therapy
• Diabetes management
• Many other post-acute services

The clinical staff and therapists work together with our physicians to develop a customized program for each individual.

At Concordia, we have spacious Physical and Occupational Therapy gyms with state-of-the-art equipment available in conjunction to one-on-one, hands-on therapy treatment. Our dedicated short-term rehabilitation team is in place to motivate, support, and increase the overall functioning of each patient. Our objective is to meet the needs of those individuals with the goal of getting them back to life as quickly as possible.

To enhance recovery, Concordia’s dietary staff provides delicious and nutritional meals and our activity staff provides therapeutic recreational activities. Cable TV service is provided in every room and many are equipped with free Wi-Fi Internet capabilities.

We encourage our skilled nursing and short-term rehab patients to participate in their daily care and to be proactive in their recovery. Our in-house social services staff is available to meet with patients and their family in order to stay informed of their progress and discharge plans. Concordia at Cabot Skilled Nursing even has an independent Family Council that meets regularly to discuss a variety of topics.
