Concordia Visiting Nurses is a community-based, non-profit, home health agency based in Cabot, PA since 1997. We currently serve Allegheny, Armstrong, Beaver, Butler, Mercer, Washington, and Westmoreland counties, but we are expanding our service area everyday. Our goal is to deliver quality home health services, in good faith, on behalf of those who entrust themselves to our care, and to minister to the unique needs of each individual with respect, dignity and compassion. Our team includes registered nurses, medical social workers, physical, speech, and occupational therapists, intravenous, and wound care nursing, and home health aides.

We offer both private and professional services designed to promote healthy habits.

Professional Services
Our professional services division is a Medicare certified, state licensed, Medicaid approved home care agency. We also participate in most private insurance plans. Please call for the current listing.

Concordia Visiting Nurses Professional Services provides expert care for patients and clients with many types of conditions, including:

Concordia Visiting Nurses provides:

• Injections
• IV Therapy
• Lab specimen collection with assessment of values
• Ostomy care
• Physical and occupational therapy
• Speech/Language pathology
• Wound care

Private Duty Services
Private duty services are typically an out-of-pocket expense. Occasionally, if you qualify financially and clinically, the local Area Agency on Aging, the VA, and several other governmental agencies will cover the cost of some services. In addition, some individuals have Long Term Care insurance that will also cover in-home care.

Private duty services include:

• Assistance with ambulation (walking) and transfers
• Card writing and gift-wrapping
• Feeding and meal preparation
• Light housekeeping
• Medication reminder
• Personal care, including bathing, grooming, and dressing
• Shopping and errands
• Other services to assist you with independence
