Each of our PA facilities has different visitation policies, based on recommendations from various governmental agencies. Please visit the website of each location to view its Reopening Plan, as mandated by the governor’s office. At ALL Concordia locations, the following policies are in effect:

  • Visitors of patients/residents in end-of-life situations are permitted. Please call the facility before coming to the building.
  • Anyone entering a building for any reason, will be screened for temperature, signs and symptoms of COVID-19, recent travel destinations and contact with someone with a confirmed or potential case of COVID-19.
  • All locations are offering alternate methods of visitation. Loved ones are encouraged to contact the Activities Department at the facility to schedule.

Legacy Society

The Legacy Society of Concordia Lutheran Ministries allows individuals to participate in shaping tomorrow's long-term health care for senior adults by making estate or life income gifts supporting Concordia’s mission.

By joining the Legacy Society, you both leave a lasting witness to your faith and elevate Concordia to the status of “family” by serving those in need for generations to come. Your bequest will fully qualify for an estate tax charitable deduction.

Members of the Legacy Society enjoy special benefits, including a welcome token of appreciation, our annual appreciation luncheon, invitations to special events and updates on Concordia's growth and expanding ministries. They also receive a yearly gift as a symbol of our gratitude. Most importantly, Legacy Society members share the pride and satisfaction of participating in the advancement of Concordia and its mission to provide the highest quality faith-based senior care.

Concordia Giving Tree (Cabot Location)

Located outside the Good Shepherd Chapel in the Lund Care Center on the Cabot campus is Concordia's Giving Tree. This memorial provides several levels for donors to celebrate, honor or memorialize a loved one:

A. Copper Leaf - gift of at least $500
B. Brass Leaf - gift of at least $1,000
C. Silver Leaf - gift of at least $5,000
D. Dove Memorial - gift of at least $10,000

Laurel Memorial Tree (Good Samaritan Hospice)

Located inside the Good Samaritan Hospice House in Wexford is the Laurel Memorial Tree. This memorial provides family and friends of our Hospice patients the opportunity to pay tribute to their loved ones.

To be eligible for a leaf on the Laurel Memorial Tree, at least $500 in unrestricted gifts must be made to the Good Samaritan Hospice Memorial Fund in honor/memory of your loved one. Multiple donors may collectively contribute toward the leaf.

A variety of permanent naming opportunities are available to support the people, programs and facilities that combine to make Concordia the most special and highest-quality senior care organization in the region. Concordia Lutheran Ministries is a qualified charity listed under section 501(c)(3) of the Internal Revenue Code. Donations to Concordia qualify for the charitable contribution deduction on federal income tax returns, subject to limits as established by the Internal Revenue Service.
