What to Expect

How does hospice care begin?

You may be familiar with hospice care, but do you know how to actually start the admission process? Hospice care typically starts with a phone call from the patient’s physician, a hospital or other health care staff member; but a family member or the patient him/herself can call us for an evaluation without a referral. Like most people, you may be unsure about when to initiate hospice services for your loved one. Who pays for hospice care?

Hospice services are covered 100% by Medicare, Medicare HMOs, and Medical Assistance and are frequently covered by private insurance plans (including UPMC and Highmark). Nursing care, durable medical equipment and some medications are also covered by Medicare. We’re happy to explore “Out of Network” benefits upon request.

Disease Management and Specialty Services

For individuals with certain chronic diseases, Concordia Visiting Nurses has established a Disease Management Program with “clinical pathways” designed to help you maintain and manage your health. A clinical pathway is a set of evidence-based practices for a specific group of patients with an oftentimes predictable clinical course, in which healthcare tasks are defined, sequenced and carried out during home health care visits. Patient outcomes are tied to following these specific series of tasks.

Our Disease Management Program provides clinical pathways for many chronic diseases like Congestive Heart Disease, Chronic Obstructive Pulmonary Disease (COPD), Diabetes, Palliative Care and more. Additionally, our program addresses Mental Illness and Orthopedic Rehabilitation.

Congestive Heart Disease

The goal of the Concordia Congestive Heart Disease (CHF) Disease Management Program is to enable clients to achieve their optimal level of health and wellness in the comfort of their own home.

Education includes:

  • Symptoms of CHF
  • Diet and nutrition
  • Exercise and activity
  • Safety and fall prevention
  • Improving quality of life
  • Energy conservation
  • Proper medication use
  • Proper equipment use
  • Breathing exercises
  • Use of “Stoplight Tool” and weight chart

Initial two-week visits include the coordinated visits of skilled nursing and occupational therapy services.

TeleHealth monitoring provides professional assessment and monitoring to improve health and outcomes, reducing re-hospitalizations.

 

The physician may also choose to order a medical social worker and home health aide to further assist their patient.

Chronic Obstructive Pulmonary Disease

The goal of the Concordia Chronic Obstructive Pulmonary Disease (COPD) Management Program is to enable clients to achieve their optimal level of health and wellness in the comfort of their own home.

Education includes:

  • Safety and fall prevention
  • Improving quality of life

A multidisciplinary team approach is utilized to help improve health and outcomes, and reduce re-hospitalizations.

The medical social worker is often helpful in addressing financial and social issues. The mental health nurse can address reducing anxiety levels relating to the disease.

Orthopedic Excellence

Concordia Visiting Nurses has an excellent Orthopedic Program to enable clients to achieve their maximum level of independence and mobility.

The goal of our program is to assist and educate clients on safe and effective therapy practices so they are able to maintain a healthy, active lifestyle.

A physical therapist first evaluates each client, creating a customized program based on the physician’s recommendations. Their care plan will include:

  • Assessment of post-operative therapy needs
  • Assessment of pain and assistance with pain management
  • Total joint replacement education
  • Fall prevention instruction
  • Strength and mobility assessment
  • Gait training
  • Assistive device education

Mental Health

Concordia Visiting Nurses provides a Mental Health Nursing Program to address the needs of clients in dealing with a mental illness.

Technology through TeleHealth

Today, patients are discharged from the hospital much earlier, but their recovery care-needs have not changed. Patients often need help integrating the medical treatment plan into their daily lives.

TeleHealth, provided by Concordia Visiting Nurses, can help reinforce some of the treatment plans and discharge instructions from the hospital setting.

Individuality is one of the cornerstones of TeleHealth. The various vital sign alert limits are individually set for each person, and he/she has the option to check themselves as often as they desire, 24 hours a day, 7 days a week.

TeleHealth also gives concerned family members the peace of mind that a professional clinician is monitoring their loved ones on a daily basis, and communicating important information to the physician - who always has immediate access to the patient’s data. Prompt communication of abnormal results allows the physician to make subtle changes in clinical care plans before a patient’s condition deteriorates.

Who Pays for Concordia Private Care?

Services can be paid privately by any person, company, trust officer or family member for private duty services not otherwise covered by an insurance company, private duty health care insurance or long-term care insurance. In some cases, Concordia Private Care may be covered by your local Area Agency on Aging (AAA) and the PDA Waiver Program, so please be sure to check with your local AAA to see if you qualify.

Some services may also be covered by private insurance. We suggest that you contact your insurance provider for more information. We also work with the U.S. Department of Veterans Affairs, and your service may be covered if you're a veteran.
