Short-Term Rehabilitation+Many people require Short-Term Rehabilitation following a fall, surgery or hospital stay, before returning to their homes. Our staff works closely with patients and their families to ensure the best possible results, which means a smooth transition back to their home environment. Personal Care+Personal Care is residential care where individuals receive professional assistance with activities of daily living, such as bathing and personal hygiene, mobility assistance, wound care, preparation of special nutritional meals or diets, housekeeping, activities, laundry, spiritual care and more. Long-Term Nursing Care+Individuals in Long-Term Nursing Care typically require a higher level of around-the-clock care from various professionals, including a medical director, nurses, therapists, social workers, dietitians and many more. Outpatient Physical Therapy+Outpatient physical therapy helps patients living in the surrounding communities find relief from pain and regain their strength, balance, independence and mobility. Outpatient services may include individualized therapy sessions, an aquatic therapy pool, treadmills, a balance machine and multiple cardio stations for upper and lower body exercise. Adult Day Services+Adult Day Services provides a safe and secure environment for individuals with a memory-impairing ailment to feel welcome and comfortable during the day. Services include daily activities, exercise class, assistance with bathing and grooming, spiritual care visits, nutritious meals and snacks, structured toileting program and more. Child Care+The Child Care center typically provides care for children ranging from eight weeks to 10 years old, including a before- and after-school program. Children are grouped according to age: infants, toddlers, preschoolers, etc and programming is tailored accordingly. General Medicine+Concordia Physician Practice is open to anyone in the community, 18 and older. Specializing in Family Practice and Geriatrics, we accept most major insurance plans and offer convenient hours.
